In an effort to keep students and staff healthy and in school, Hardeman County School System and Hardeman County Community Health Center have partnered to bring the School Health Clinic to Hardeman County Schools. The School Health Clinic is located in Bolivar Elementary School at 445 Nuckolls Road, Bolivar, TN.

 

School Health Clinic Providers

Debbie Dillard is a Board Certified Family Nurse Practitioner with a Master's of Science degree from The University of Memphis. Debbie has 22 years of RN experience in many areas such as ER, Surgical ICU, Pediatrics, and Pediatric ICU. Debbie works five days a week at the School Health Clinic and provides a wide range of medical services for students, faculty, and staff. Debbie is a lifelong resident of Hardeman County and is a 1988 graduate of Bolivar Central High School.


Corshanta Walton is a Licensed Practical Nurse and works alongside Debbie Dillard in the School Health Clinic.

What are the goals of the School Health Clinic?
The School Health Clinic strives to promote, protect, and maintain the overall health of school students and employees of the school system. Our #1 goal is to help children stay healthy and in class. It is our mission to promote quality healthcare alongside the patient's regular healthcare provider.

What services are offered?
The School Health Clinic will treat the following:

  • Common cold
  • Cough
  • Sore throats
  • Ear ache
  • Rashes/ poison ivy
  • Flu and flu-like symptoms
  • Medical care and exams for faculty and employees
  • Sports physicals
  • School entrance physicals
  • Well-child exams (EPSDT)
  • Lab testing
  • Referrals to other sources
  • Minor injuries
  • Other illnesses

How can my child be seen at the clinic?
In order for students to be seen, a consent form must be signed by a parent/guardian. Walk-ins as referred by the school nurse and appointments can also be made by calling the School Health Clinic at 731-658-2902.

Who can be seen at the School Health Clinic?
Students and employees of the Hardeman County School System can be seen at the clinic.

Will transportation to the clinic be provided?
No. Parents/guardians will be responsible for providing transportation to and from the School Health Clinic location. In the future, students and employees will be able to be seen by telehealth at the school in which they attend or are employed.

How will I know what happened during my child's visit?
A note from the provider will be sent home with the child indicating that they were treated at the School Health Clinic. The contact information for the School Health Clinic nurse will be on the note for the parent/guardian to call for additional information. The School Health Clinic will attempt to contact the parent/guardian before or during the child's visit.
